At CHP Home Care & Hospice, we believe great care begins at home. The nursing supervisor is responsible to manage the day-to-day operations of a local home health and hospice branch office, directing staff and long-term planning of patient care. Promote and restore patients' health by collaborating with physicians and multidisciplinary professional staff, and supporting patients and families. This is a full-time, salaried position that reports directly to the Director of Nursing.

SALARY:  Competitive

HOURS:  Full Time     

LOCATION:  Position available in Bryan



Examples of duties include: 


· Establish staff-patient assignments to provide appropriate care during all service hours


· Assist staff in development of plans of care and actively involved in patient care as needed


· Responsible for reviewing all admissions, implementing plan of care, and assuring all are within the scope of home care regulations


· Monitor patient progress, coordination of services, expected outcomes, and assist staff in determining time of discharge


· Oversee private duty home care and hospice programs


· Interview and select new employees as delegated


· Direct and evaluate personnel administering both positive reinforcement and constructive criticism and discipline


· Represent the corporation at community functions and participate in speaking engagements to organizations, physicians, and potential referral sources






· Graduate of accredited school of nursing


· Currently licensed in the state of Ohio


· Physically capable to lift and transfer patients


· Maintain CPR certification and License


· Excellent communication skills


· Reliable transportation and proof of current driver’s license and insurance






· Registered Nurse with at least five years experience - home health experience preferred


· Supervision, Nursing Skills, and Good Judgment
